If this is true, I don’t think the man should have been reassigned. The person responsible for accepting and maintaining the software should have been let go.

That’s just a horrible user interface and probably not limited to that single drop down menu. It was a accident waiting to happen and they should have seen it coming.

That mess up was much bigger than the guy that made the wrong selection.

My guess is that someone wanted someone blamed quickly so no one would look to hard.
