Indeed, Christopher, abusive behavior by subordinates means that superiors are not held accountable and, better yet, they’re promoted. This is highlighted in the news of Gen. McChrystal’s probable 4th star:

But it is unclear how many of these fell under his direct command, and McChrystal has never been sanctioned.
